That can mean a ground ambulance across a city, a helicopter off a ship or a mountain, or an air ambulance across a border. Once a doctor says you are stable enough to travel, it also pays to fly you home, with a medical escort or on a medically equipped flight if you need one. If you die on the trip, it pays to return your remains to your family. All of it is arranged by the insurer’s 24-hour assistance company, which is why the first call is always to the number on your policy.
Two words come up on every policy. Evacuation is getting you to adequate care. Repatriation is getting you home. Most plans put both under one limit.
What an evacuation costs without it
An air medical transport runs from about $30,000 to over $250,000, according to Medjet, which arranges them for a living. The price depends on the distance, the aircraft and the medical crew on board. A helicopter from a cruise ship to a Caribbean island hospital is at the low end. A jet from Southeast Asia or Africa to the United States with a nurse and a doctor is at the high end. Your health insurance from home does not pay for any of it, and neither does Medicare.

Notice the Allianz line. OneTrip Premier carries a strong $1,000,000 of evacuation and only $75,000 of medical, so a long hospital stay abroad would run out of medical coverage before the evacuation benefit was ever touched. Price a plan on both numbers, not one.

The cheapest plan on a quote is cheap for a reason. These limits will not cover a single air ambulance from most of the world.

There are two ways to buy evacuation coverage, and they do different jobs.

For most trips the package plan is enough, because the assistance company will move you to adequate care and get you home. A membership is worth adding when being treated at your own hospital matters more than anything else: a serious condition, a long trip through countries with thin medical care, or a family member who would insist on it. Medicare pays nothing for care or transport outside the United States, apart from a few narrow border cases, and on a cruise it stops paying once the ship is more than six hours from a US port. Some Medicare Advantage and Medigap plans pay part of a foreign hospital bill up to a lifetime cap, but none of them pay for an evacuation. Employer and marketplace health plans vary, but even the ones that pay something abroad treat you as out of network, want the bill paid first and pay nothing to fly you anywhere. How much evacuation coverage you need
$250,000 is a sensible floor for a trip abroad. $500,000 or more for a cruise, a mountain or safari trip, or anywhere more than a few hours from a major hospital. Because the top plans carry $1,000,000 for about the same premium as plans with a fraction of that, there is rarely a reason to accept less. How to make the benefit work when you need it
- Call the assistance number first. The insurer arranges and pays for the transport. A transport you arrange yourself may be reimbursed only up to what the insurer would have paid, or not at all.
- Let the doctors and the assistance company decide the destination. On a package plan the benefit is for the nearest adequate hospital, then home once you are stable. Asking to go straight to your own hospital is what a membership is for.
- Keep the policy number and the phone number on your phone and in your wallet. The person calling is often not you.
- Tell the insurer about any condition before the trip if you are unsure. A condition that changed inside the look-back period can be excluded unless the plan’s pre-existing condition waiver applies, and the evacuation for it would be excluded too.
