The following are my recommended insurance plans for group trips based on the criteria listed above, individual features in each policy, and the cost. All plans are offered by A.M.Best Rated reputable companies that specialize in travel insurance.
This plan is perfect for a large family, or a multi-generational trip that includes grandparents, adults, and the grandchildren. It is a standard plan (covers cancellation, interruption, medical, evacuation, baggage, delays, etc) that you would purchase for any other trip. However, it happens to have great pricing for a larger traveling group. Quoting/purchase process is just like any other travel insurance plan. Note: All travelers must reside in the same state.
Example quote $816 (includes all travelers): For the above plan, the example trip includes 10 travelers (70, 70, 45, 45, 40, 40, 14, 14, 12, 12) on a two week trip, with a total trip cost of $20,000).
For groups of more than 10 travelers that still want full comprehensive coverage for their trip, the following plan is the best choice. This is a plan that has special group rating for larger groups, and simplified enrollment. This is a good plan for larger families and organized tours, or any other trip where you want comprehensive coverage (covers cancellation, interruption, medical, evacuation, baggage, delays, etc). Example quote $2,014 (includes all travelers): For the above plan, the example trip includes 16 travelers (70, 70, 70, 70, 45, 45, 42, 42, 40, 40, 14, 14, 14, 14, 12, 12) on a two week trip, with a total trip cost of $32,000).
For a group trip larger than 10 travelers where you only want coverage for medical emergencies, this is the best plan. With comprehensive travel insurance, you have coverage for canceled or interrupted trips, medical emergencies, baggage, etc. This plan focuses on emergency medical coverage and medical evacuation coverage. This is a simpler, lower-cost option for when cancellation is not a concern.

In general, comprehensive travel insurance costs between 4-10% of your insured trip cost. A travel medical only plan can vary, but a general rule would be under $10/day for each traveler.
Comprehensive plans cost more because they are insuring your trip cost in addition to medical emergencies. That is a lot of financial risk for the insurance company, so they need to charge more. The same trip with medical only would have less financial risk, so far less premium.
